Voluntary Sector Discharge from Hospital Support Worker
30 hours a week
Salary currently : £30,646.98 pa pro rata
Based at Torbay Hospital
We are looking to recruit a 2nd team member to join our Hospital Discharge Support Programme which aims to support patients, carers and families on discharge from Torbay Hospital. Working on the wards at the Acute Hospital in Torquay with nursing and ward staff, the aim is to support timely discharge, improved wellbeing and self-care through introduction to voluntary and community support services.
We are looking for a compassionate, organised and dedicated person who is curious and empathetic, non-judgemental, is great at listening and responding and is able to motivate and inspire patients to engage with the support.
