Moorland Community Care Group - Wellbeing Coordinator (2 posts)

Required as soon as possible:

Wellbeing Coordinator (Dementia Support) part-time: 20 hours per week
Wellbeing Coordinator (End of Life Support) part time: 10 hours per week

Both of these posts are fixed-term contracts for 18 months, salary scale £19,129 per annum pro rata. Candidates may apply for both posts if they wish, as the hours can be worked flexibly to enable them to be undertaken by one individual.

The charity works with lonely isolated and vulnerable adults providing social care, and has expanding its role within the NHS to provide guided help so that they can be proactive in managing their own wellbeing; reduce their need for urgent and emergency services by improving their experiences within the community and being helped to focus on the things that matter to them and access community resources. These two new posts have been created to focus on clients with the specific needs as indicated above. The post holders will be expected to work with the existing Wellbeing and Lead Wellbeing Coordinators, but also independently, and will be ultimately responsible to the Board of Trustees.

Applicants should ideally have some experience of working with elderly and vulnerable people, and also with volunteers, although specific training in the designated field(s) of responsibility will be provided in conjunction with Teignbridge Community and Voluntary Services. You must have good interpersonal skills, appropriate IT skills, be efficient at keeping records, and be aware of the need for confidentiality when dealing with clients. You must also be a car owner with a current driving licence.
